Alzheimer’s Society Support

We are pleased to announce we are working alongside The Alzheimer’s Society to increase support for our patients, who have dementia or may be waiting on diagnosis, and their carers, friends and family.  

The  Alzheimer’s Society will be attending the practice for a support session on the first Tuesday of every other month, with the next session being the 4th of October.

If you would like to attend please contact us on 01354606300 option 1 and we will be happy to book you in.

Outside of these sessions, their Dementia Connect Service is available for help and support.

 

Dementia Connect, Alzheimer’s Society’s dementia support service, is free, easy to access, and puts you in touch with the right support. From local help to phone and online advice.

Dementia advisers are available on the phone. Local dementia advisers can go out and meet you in your community to offer further support

Here are a few examples of the support they can provide:
■ help with legal and financial documents
■ emotional support when things get tough
■ connecting you to local support groups
■ help to understand and live with dementia, including
coping techniques
■ support with everyday living, such as government benefits.

What happens next?
A Dementia Adviser will talk to you about what you need and
connect you to the right support for you. And you’ll only need
to tell your story to us once.
